Please read the following to understand the Douglas Williams Executive Health Program's ("EHP") views and practices regarding privacy and how they pertain to users of the EHP's web site. The EHP is strongly committed to protecting the privacy of patients (also referred to as "Users" in this Privacy Policy) including those who use its interactive products and services. At the EHP, we understand that your health information is very personal and private. The EHP has created this privacy statement in order to demonstrate our firm commitment to you and your privacy.
The EHP may, from time to time, change the features and content found on its Site. For example, regulations are expected to be issued under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"). The HIPAA regulations are expected to contain rules governing the use, storage and disclosure of protected health information stored or transmitted in electronic form. The EHP uses personal information primarily as an integral part of establishing a comprehensive personal medical history from which our medical staff will determine appropriate health assessment regimens. In order to provide you with a more personalized and relevant web service, The EHP also collects and uses personal information to identify your specific interests and needs. How and what information does the EHP gather or track? Through the use of on-line data entry, survey, and registration forms, the EHP may collect and/or monitor, in aggregate and individual form, personal identifying facts or data on, among other things, your e-mail address. The EHP obtains most of the personal information from patient records provided to and gathered at our University of Florida location. Does the EHP disclose personal information to third parties? Under no circumstances will the EHP release any of your personal identifiable information stored in our database to any person or company without your consent, unless otherwise required by law or ordered by a court of law. We use state-of-the-art technology to ensure that your information can be stored and retrieved securely and safely.
