The EHP uses personal information primarily as an integral part of establishing a comprehensive personal medical history from which our medical staff will determine appropriate health assessment regimens. How and what information does the EHP gather or track? Through the use of on-line data entry, survey, and registration forms, the EHP may collect and/or monitor, in aggregate and individual form, personal identifying facts or data on, among other things, your e-mail address. The EHP obtains most of the personal information from patient records provided to and gathered at our University of Florida location.

Does the EHP disclose personal information to third parties?
